When you come for your first appointment, don't forget to bring the following:

• Drivers License or other valid ID

• Insurance information

• Referral Letter (if applicable)

• Reports, X-rays, MRIs, CT scans etc and any other relevant information

• List of medications (if any)

If you need to cancel your appointment with Dr. Scott, please call the office during business hours at least 24 hours in advance so that we can offer your appointment time to another patient waiting to be seen.

We know that your time is valuable. We make every effort to run on time but occasionally patients require a little more time. Every patient deserves the full attention of their provider and should not feel rushed during their appointment. We therefore apologize if we keep you waiting.
